Do you need our services?

Everyone that is interested in becoming a client will need to come to our clinic in person during our open hours and fill out an application.

The application to become a client is dependent on income and where you live. Therefore, we will need you to provide proof of income and zip code as part of your application. This can simply be a paystub/paycheck, or it can be an award letter of aid (Medicare, Social Security, food-stamps etc). We need letters from ALL aid services you receive.
